The following items are required in order for your child to participate:

• 2023 Registration form

• 2023 Pop Warner contract/waiver completed and signed by parent and participant – 2 pages

• Photo Release

• Hold Harmless parent agreement

• ­­­­2023 Pop Warner Physical form completed front by parent and back by doctor (APRN, MD, DC, DO)

• COVID 19 Waiver

 

Items needed from parent:

• Report Card – 2 copies of FINAL (2022-2023) report card with all four quarters and final grade

  • ​This can be located under the portfolio section on Skyward for most Lake County Schools

• Photo – Current headshot emailed to: ClermontKnightsRegister@gmail.com with child’s name

• Birth Certificate – Copy of state issued certificate / original to be shown for verification

• Health Insurance card – Copy of front and back

• Registration payment in full
